  4. Trainer resigning from current practice <\/li>\n\n\n\n
  5. Trainer moving to new practice <\/li>\n\n\n\n
  6. Change of Practice Manager <\/li>\n\n\n\n
  7. Changes to premises <\/li>\n<\/ol>\n\n\n\n
